Nursing Home Award Program 

 

We know you care about your residents and want to provide them with the best quality of life possible. We’re offering you an opportunity to improve your residents’ lives—and to provide recognition for your nursing home in the process.

 

How does my home get involved?

Each nursing home must fill out an application for nomination where they will select two topic areas, one clinical and one non-clinical, on which to work. One of the topic areas must be an Advancing Excellence in America’s Nursing Homes goal. More information on participation can be found by clicking on "Application" on the left. Participants must fill out a mid-point progress report and final progress report as a condition of participation.

 

What determines an award winner?

Homes that show positive outcomes in both selected topic areas may qualify for an award as long as they are also a member of the Advancing Excellence Campaign and have in place an active quality improvement process. More detailed information on award criteria can be found by clicking on "Award Criteria" on the left.

 

How do I sign up?

Fill out the application for nomination and choose your two topic areas. The nomination period began November 1, 2007.

 

What should I do if I have more questions?

If you have more questions or are just curious about the Award Program, please attending the Q&A WebEx, being held Tuesday, November 13 from 11:00 a.m. - 12:00 p.m. More information can be found on the Events page.
