Award Criteria    

Type of Awards

Two levels of awards will be given: Quality Achievement Award and Award of Excellence.

1.      Quality Achievement Award Criteria

Given to nursing homes that meet the criteria listed below:

    • Be a member of the Advancing Excellence in America’s Nursing Homes Campaign
    • Have no citations for deficiencies of Scope & Severity “F” or higher from October 1, 2007 through December 31, 2008
    • Have in place an active quality improvement process for the selected measures, verified by review of a mid-point progress report
    • Achieve positive outcomes in both selected areas of focus, which can be verified by the final report and/or an on-site visit, should the NHQIC choose

2.      Award of Excellence Criteria

Given to nursing homes that meet all of the criteria for the Quality Achievement Award and have met the following additional requirements:

    • Have verification of contract with a Certified Medical Director (CMD), which can be provided during on-site visit
    • Have documentation of a functioning innovative quality improvement process for the measures selected (verified during an on-site visit)
    • The nursing home will be transparent with their data in the Advancing Excellence in America’s Nursing Homes Campaign
    • Have no citations for deficiencies of Scope & Severity “E” or higher from October 1, 2007 through December 31, 2008
