Select at least one clinical topic and one non-clinical topic on which you will focus on improving (with at least one being an Advancing Excellence in America’s Nursing Homes goal)

Establish a multidisciplinary team to focus on selected areas, and develop a schedule for regular meetings to discuss progress

Identify a project leader for correspondence on progress related to the selected topics

Review the mid-point progress report summary 

Create a binder for documentation of Award Program activities and focus area interventions and progress

Determine a measurement system for each focus area and establish a process for collecting progress data, indicating where interventions were implemented on the timeline of progress

Review the two types of awards and their criteria

Consider attending the informational WebEx to ask any questions about the Award Program
