Responsibilities    

Responsibility of Participants

To be eligible for consideration, nursing homes must:

  1. Nominate their home by submitting an application for nomination electronically on this Web site. (One application per nursing home.)

  2. Be a member of the Advancing Excellence in America’s Nursing Homes Campaign.  To become a member go to http://www.nhqualitycampaign.org.

  3. Select a minimum of two areas on which to focus quality improvement efforts (one clinical and one non-clinical).

  4. Establish a multidisciplinary team to focus on their selected areas, and identify a project leader for correspondence on progress related to the selected topics.

  5. Determine a measurement system for each focus area and establish a process for collecting progress data, indicating where interventions were implemented on the timeline of progress.

  6. Complete and submit a mid-point progress report electronically identifying:
    1. Two selected areas of focus
    2. What interventions have been tested thus far
    3. Interim performance measurement data, related to the selected topic areas

  7. Create a binder to document Award Program activities and focus area interventions and progress.

  8. Submit a final progress report electronically highlighting changes made and quality improvement outcomes achieved throughout the award program; in addition, applicants will explain their plans for sustainability of improvements.

  9. Agree to public disclosure of participation in the award program.

  10. Be in substantial compliance at the time the award is given.
